How much is Eliquis with Medicare in Arizona in 2027?
Medicare's negotiated price for a 30-day supply of Eliquis is $237.25 in 2027, and with Part D your own cost depends on your plan's deductible (no more than $700) and tier, with all covered drug costs capped at $2,400 for the year.
Prices are national Medicare figures from CMS, and the deductible range is from the 2027 plan files for Maricopa County. Eliquis is used to prevent blood clots and stroke in people with atrial fibrillation and to treat or prevent DVT and PE. What you pay at a pharmacy in Chandler, AZ or anywhere in Arizona depends on your 2027 plan's deductible, the tier Eliquis sits on, and whether you have reached the yearly cap.

How Part D pricing works for this drug in 2027
Your plan pays the pharmacy a negotiated price, and your share comes in three steps, in this order:
Step 1, the deductible: you pay the plan's full price until your deductible is met. No 2027 plan may set it above $700 (it was $615 in 2026). The 85 Maricopa County plans with drug coverage run from $0 to $700, and Arizona's 9 stand-alone drug plans from $300 to $700.
Step 2, your tier: after the deductible you pay the copay or coinsurance set by the tier Eliquis sits on. Under the standard design CMS publishes, that is 25% of the drug's cost.
Step 3, the yearly cap: once your out-of-pocket spending on covered drugs reaches $2,400 in 2027 (up from $2,100 in 2026), you pay $0 for covered Part D drugs for the rest of the year.
Worked year: 12 monthly fills at the $237.25 negotiated price add up to $2,847 in drug cost. On a standard-design plan you pay the first $700, then 25% of the remaining $2,147, about $1,236.75 for 2027, before any other drugs you take. Plans with copays or a lower deductible come out differently, which is why we price your own list.
What changes the answer
Eliquis was one of the first 10 drugs with a Medicare negotiated price. CMS set it at $231 for 2026 and adjusted it for inflation to $237.25 from January 1, 2027, against a $521 list price. The effect shows in CMS data: the average gross cost per Part D claim fell from $912.31 in 2025 to $399.01 in the first quarter of 2026.
For scale, 4,806,752 people with Part D filled Eliquis in 2025, at an average gross cost of $912.31 per claim (national CMS data; a claim can be a 30-day or 90-day fill). Formularies are set plan by plan, so the tier and any prior authorization on your own 2027 plan decide your copay. People with limited income can apply for Extra Help through Social Security, which lowers drug copays.
What to do next
Bring your prescription list to a plan review. During Annual Enrollment, October 15 to December 7, 2026, we compare how Eliquis is covered on the 2027 plans we offer in Maricopa and Pinal counties, including tier, deductible and pharmacy, and show you the yearly total rather than one copay.

Is Eliquis covered by Medicare in 2027?
Eliquis is covered under Medicare Part D drug plans and Medicare Advantage plans with drug coverage when it is on the plan's 2027 formulary. Coverage, tier and any prior authorization vary by plan.
What is the most I can pay for Part D drugs in 2027?
$2,400 for covered Part D drugs, up from $2,100 in 2026. After that you pay $0 for covered drugs for the rest of the year.
Does the Part D deductible apply to Eliquis in 2027?
It can. Until your plan's deductible is met you may pay the plan's full price for Eliquis. No plan may set a 2027 deductible above $700, and Maricopa County drug plans range from $0 to $700, so where Eliquis sits on each plan's 2027 drug list matters as much as the premium.
Can Extra Help lower what I pay for Eliquis?
Yes, if you qualify. Extra Help is a federal program, applied for through Social Security, that lowers Part D premiums, deductibles and copays for people with limited income and resources. In Arizona, people with full AHCCCS coverage or a Medicare Savings Program usually get it automatically. We can help you check.
Can I spread my Eliquis costs over 2027?
Yes. The Medicare Prescription Payment Plan lets you pay your out-of-pocket Part D drug costs in monthly amounts across the calendar year instead of all at once at the pharmacy. It does not lower the total you pay, but it helps when the deductible lands in January. Ask your plan to sign you up.
How do I check whether my 2027 plan covers Eliquis?
We do it with you. During Annual Enrollment, October 15 to December 7, 2026, we look up Eliquis on the 2027 drug list of each plan we offer in Maricopa and Pinal counties, note the tier and any prior authorization or quantity limits, and show you the yearly total.
Will I pay the Medicare negotiated price for Eliquis at the pharmacy?
Only while you are in your deductible. The $237.25 negotiated price is what your plan's 2027 cost for a 30-day supply is based on; after the deductible you pay your tier's copay or coinsurance until the $2,400 yearly cap stops your costs for covered drugs.
Did the Medicare price of Eliquis change for 2027?
Slightly. CMS's inflation adjustment moves the negotiated price for a 30-day supply from $231 in 2026 to $237.25 from January 1, 2027. Your copay still depends on your 2027 plan.
