Chronic condition (C-SNP): 42
For people with a qualifying condition such as diabetes, chronic heart failure or cardiovascular disease. A doctor confirms the condition.

Arizona has 89 Medicare Advantage plans for 2027, but no single county has them all. Which ones you can join depends on your county, and the counts run from 52 in Maricopa County to 7 in Apache County.
Plans are approved by county, so this table is the real starting point. Counts exclude Special Needs Plans, which are in the last column.
The urban counties carry most of the choice. Maricopa and Pima counties each have 52 Medicare Advantage plans for 2027 and Pinal County has 47, while most rural counties have fewer than 20. 4 counties gained plans from 2026 and 9 lost some. In Apache County, Greenlee County, and Navajo County, every Medicare Advantage plan on the 2027 list is a PPO.
Two terms in the table: an HMO plan generally covers care only from doctors and hospitals in its network, and you may need a referral to see a specialist. A PPO plan also has a network, but it pays part of the cost when you see a provider outside it. The "$0 medical premium" column counts plans that charge nothing for their medical coverage; some of them still charge for drug coverage, and every member keeps paying the Part B premium.
| County | Plans 2027 | Plans 2026 | $0 medical premium (Part C only) | Carriers | HMO / PPO | Out-of-pocket limits | Special Needs Plans |
|---|---|---|---|---|---|---|---|
| Maricopa County | 52 | 52 | 50 | 13 | 36 / 16 | $2,000 to $8,900 | 40 |
| Pima County | 52 | 50 | 50 | 12 | 33 / 19 | $1,899 to $8,900 | 36 |
| Pinal County | 47 | 46 | 45 | 10 | 31 / 16 | $2,000 to $8,900 | 24 |
| Yavapai County | 20 | 21 | 18 | 3 | 7 / 13 | $4,150 to $9,450 | 11 |
| Graham County | 19 | 21 | 16 | 4 | 8 / 11 | $4,000 to $9,450 | 13 |
| Santa Cruz County | 17 | 20 | 14 | 6 | 6 / 11 | $2,499 to $9,800 | 11 |
| Yuma County | 17 | 16 | 15 | 4 | 6 / 11 | $4,150 to $9,800 | 11 |

| Gila County | 9 | 11 | 7 | 3 | 1 / 8 | $4,150 to $9,800 | 13 |
| Greenlee County | 8 | 11 | 6 | 2 | 0 / 8 | $4,150 to $7,850 | 7 |

"$0 medical premium" is the Part C premium alone, not the drug premium. Out-of-pocket limits are the lowest and highest in-network limits among that county's plans.

Each plan is counted once, however many counties it serves. A company with many plans is not better or worse; what matters is whether a plan covers your doctors and drugs.

A Special Needs Plan is a Medicare Advantage plan limited to people who meet a condition, so it is counted apart from the plans above.
For people with a qualifying condition such as diabetes, chronic heart failure or cardiovascular disease. A doctor confirms the condition.
For people with both Medicare and AHCCCS, Arizona Medicaid. AHCCCS decides eligibility; the plan coordinates both programs.
For people who live in, or need the level of care of, a nursing home or similar facility.

Stand-alone Part D plans are sold by region, and Arizona is one region, so the same 9 plans are available in every county. People usually pair one with Original Medicare and a Medigap policy. Most Medicare Advantage plans include drug coverage instead.
The rules are national. In 2027 the standard deductible is $700 (up from $615 in 2026) and the yearly out-of-pocket cap is $2,400 (up from $2,100), both from the CMS CY2027 Rate Announcement, Attachment V, which lists the 2026 and 2027 amounts side by side. Arizona's stand-alone plans set deductibles of $300, $350, $687, and $700 (CMS Landscape 2027).
The plan that fits depends on your own prescriptions and pharmacy, so Viola checks each drug against the 2027 drug lists. Read more in 2027 Part D formularies: is your drug still covered? and what the 2027 out-of-pocket cap means.
AHCCCS, Arizona Medicaid, runs the Medicare Savings Programs. Effective February 1, 2026, a single person with gross monthly income under $1,330 ($1,804 for a couple) can have Part A and Part B premiums, copays and deductibles paid, and under $1,796 ($2,435 for a couple) can have the Part B premium paid, according to AHCCCS. AHCCCS makes the decision; we help you choose a plan that works with it.
Arizona has no state birthday or anniversary rule for Medicare Supplement policies: its Medicare supplement statute, A.R.S. 20-1133, creates no such right. Your best window is the federal 6-month Medigap open enrollment period that starts when you are 65 or older and enrolled in Part B; inside it, an insurer cannot turn you down or charge more for your health (CMS, Choosing a Medigap Policy). See which states have a Medigap birthday rule and Medigap switching rules in Arizona.
Arizona uses HealthCare.gov, the federal Marketplace, for individual and family coverage. Open enrollment for 2027 coverage runs November 1, 2026 to January 15, 2027, with December 15 the last day to choose a plan that starts January 1 (HealthCare.gov dates and deadlines), and we enroll you there. Rise Up Benefits Group is not affiliated with, endorsed by, or operating on behalf of HealthCare.gov, the Health Insurance Marketplace, or any federal or state government agency. Plan availability, premiums and advance premium tax credits are set by the carriers and the Marketplace.
Outside Maricopa, Pima and Pinal counties, the choice between the two paths looks different. In Apache County (7 plans), Greenlee County (8 plans), Navajo County (8 plans), and Mohave County (12 plans), only one or two insurance companies offer Medicare Advantage for 2027. The highest in-network out-of-pocket limit in the other 12 counties is $9,800, against $8,900 in the three metro counties (CMS Landscape 2027).
A thin network matters most when the nearest specialist or hospital is a long drive away, or across a county or state line. A Medicare Supplement (Medigap) policy works with any doctor or hospital in the country that accepts Medicare, with no referrals; only Medicare SELECT policies use a network (Medicare.gov, what Medigap covers). You pay a monthly Medigap premium plus a stand-alone Part D plan, so the monthly cost is higher; the trade is fewer surprises when care is far from home.
Many Arizonans spend part of the year in another state or in Mexico. Medicare Advantage networks are local, so routine care away from home may cost more or not be covered; emergency and urgent care are covered anywhere in the U.S. (Medicare & You). Outside the country, Original Medicare generally pays nothing, while several Medigap plans pay 80% of emergency care after a $250 deductible, up to a $50,000 lifetime limit, for the first 60 days of a trip (CMS fact sheet 11037). Read travel coverage for Arizona snowbirds for the details.
One timing rule decides a lot: outside your Medigap open enrollment window, Arizona insurers can ask health questions before selling you a policy, so leaving Medicare Advantage for Medigap later is not guaranteed. Viola checks that timing with you before you pick a path.
Every Arizona review starts the same way, whether you live in Chandler or Flagstaff. First the county on your address, because it decides which of the 89 plans you can join. Then the doctors and hospitals you actually use, checked by name in each plan's 2027 directory, and every prescription, dose and pharmacy against the 2027 drug lists.
Next comes whether you may qualify for help from AHCCCS or Extra Help, which can change the right plan entirely. Only then does Viola compare premiums, out-of-pocket limits and extras, so you know what you have, how to use it and what it covers. In person at the Chandler, AZ office, or by phone or video anywhere in Arizona, and a spouse or family member is welcome at every step.
Viola, a Certified in Long Term Care (CLTC) agent, founded Rise Up Benefits Group in 2019 and is licensed in 29 states plus DC. Every Wednesday, at 11:00 AM and 5:30 PM, the agency holds a Medicare workshop of about 90 minutes at the Chandler office. It is educational only: no plans are sold and there is no obligation to enroll. See the seminar schedule.
Each guide pairs its county's plan list with that community's own Census, CDC and hospital figures.
October 15 to December 7, 2026. Join, switch or drop a Medicare Advantage or Part D plan; changes start January 1, 2027.
January 1 to March 31, 2027. If you are in a Medicare Advantage plan, you can switch once or return to Original Medicare.
Seven months around your 65th birthday: the 3 months before, your birthday month and the 3 months after.
A move to another Arizona county, losing employer coverage, gaining or losing AHCCCS, or a plan leaving your county can open a window at other times.

The CMS 2027 Landscape file lists 89 different Medicare Advantage plans across Arizona's 15 counties, up from 83 in 2026, not counting Special Needs Plans. No county has all of them: plans are approved county by county, from 52 in Maricopa County down to 7 in Apache County. Viola starts every review with your county, then narrows that list to your doctors and prescriptions.
CMS approves each Medicare Advantage plan for a service area made of whole counties, so the county on your home address decides which plans you can join. A move to another county can change your choices and opens a Special Enrollment Period. Tell us your ZIP code and we confirm the county list before comparing anything.
No. Arizona has no state birthday or anniversary rule for Medicare Supplement policies, so the federal rules apply: your 6-month Medigap open enrollment window, plus federal guaranteed-issue rights in specific situations, such as losing other coverage. Outside those, an insurer in Arizona may ask health questions. We check your timing before you apply so you do not lose a right you have.
It is $2,400 in 2027, the same in every state, set in the CMS CY2027 Rate Announcement (it was $2,100 in 2026). Once your covered drug costs reach the cap, you pay nothing more for covered Part D drugs that year. We check each of your prescriptions against the 2027 drug lists so you know how quickly you would reach it.
AHCCCS, Arizona Medicaid, decides who qualifies for Medicaid and the Medicare Savings Programs. Once you know your status, a Dual Special Needs Plan may fit, and Viola compares the D-SNPs on your county list with you, checks your doctors, and handles the enrollment. Applying to AHCCCS is free; our help choosing a plan has no cost to you.
Neither is better for everyone, but the trade is sharper outside the Phoenix and Tucson areas. In Apache County, Greenlee County, Navajo County, and Mohave County, the 2027 CMS Landscape file shows only one or two insurance companies offering Medicare Advantage, so the network question comes first. A Medicare Supplement works with any doctor or hospital in the country that accepts Medicare, at a higher monthly premium. Viola lays out both paths for your county, your doctors and your budget before you choose.
